Website privacy
Your enquiry.
Your information.
This notice explains the information handled through the Extraordinary Abilities website’s general enquiry and referral enquiry forms.
Who operates this website?
Extraordinary Abilities Pty Ltd, ABN 75 639 686 365. Contact our team on 0452 590 584 or info@extraordinaryabilities.com.au with a website privacy question or concern.
What the forms collect
The general enquiry form asks for your name, email, optional phone number, role, service of interest, preferred suburb or area, a brief message and consent to contact. The separate referral enquiry form also includes an optional organisation field. We do not ask for a participant’s NDIS number, medical records or full home address through these forms.
How the information is used
Information provided through these forms is used to respond to the enquiry, discuss service availability and suitability, and consider possible next steps. Submitting an enquiry does not enrol anyone in a service or establish an agreed support arrangement.
Storage and service providers
This website uses GoDaddy Managed WordPress hosting and Forminator to process forms. Form submissions are stored in the website database and an email notification is configured for the Extraordinary Abilities enquiry inbox. Website and hosting systems may also process technical information, such as IP addresses and browser details, for website operation and security.
The website database is configured to retain these initial form submissions for 90 days. Email correspondence and any later service records are separate from that website retention setting. Contact the team to ask about the handling of information outside this website.
Cookies, links and analytics
WordPress uses cookies for functions such as authorised administrator sign-in. No marketing analytics or advertising pixels have been added to this website. External links, including Facebook, Instagram and the NDIS Commission register, lead to websites with their own privacy practices.
Access, correction and concerns
Contact us if you want to ask about information you have provided, request a correction or raise a privacy concern. Please begin with a general request and a suitable contact method rather than sending identity documents through this form.
